Do I need to transfer my prescriptions to CGH Hometown Pharmacy?
Yes. If you have existing prescriptions at another pharmacy that you would like filled at CGH Hometown Pharmacy, they will need to be transferred. CGH Hometown Pharmacy can handle the transfer process from your previous pharmacy for you.

Will my current refills automatically be available at the new pharmacy?
If your prescription is transferred to (or is currently at) the Hometown Pharmacy, simply enroll in the auto-refill program, and your prescription will be ready before you run out.

Either contact your provider’s office to have the prescription sent to the correct pharmacy, or call Hometown Pharmacy at 815-632-5371 to help transfer the prescription to the correct pharmacy.

Controlled substance prescriptions are subject to state and federal regulations that may restrict or limit transfers. Please contact the pharmacy directly, and the best course of action based on your specific medication and situation will be determined.

If your medication is temporarily out of stock, CGH Hometown Pharmacy will not only work to source it as quickly as possible, but will also help coordinate a transfer to another location to ensure you receive your medication without delay.

Refills and Prescription Pick-up

How do I request a refill?
Simply call the pharmacy or use your MyChart account/app.
To refill your prescriptions in MyChart, sign in to your MyChart account. (If no refills remain, or if the medication was prescribed by a provider outside of CGH, the request will be sent to your provider for approval):
• Select the “Medications” shortcut, or find “Medications” in the drop-down menu.
• From the medication list, click “Request Refill”.
• Select the check box next to the medication you need refilled. Click “Next”.
• Enter any comments for the pharmacist, and review the delivery method and pharmacy information. Click “Next”.
• Review the details of your refill request and click “Submit”.
• You will receive a message in your MyChart Inbox, and/or via text/email, when your prescription refill is processed.

Can I receive text notifications when my prescription is ready?
Yes. This can be set up through the pharmacy or in your MyChart account.
